เงื่อนไขการรับบริการ

1. ขอสงวนสิทธิ์การให้บริการสำหรับท่านที่นำคูปองมาแสดงเท่านั้น

2. คูปองนี้ไม่สามารถแลกเปลี่ยนเป็นเงินสดได้

3. คูปองนี้สำหรับการตรวจตามรายการที่ระบุเท่านั้น ไม่สามารถเปลี่ยนแปลงได้

4. สามารถใช้สำหรับผู้รับบริการผู้ป่วยนอก (OPD) และผู้รับบริการที่ช่วยเหลือตัวเองได้เท่านั้น

5. ราคาดังกล่าวรวมค่าห้องผ่าตัด ค่าห้องพักฟื้น ค่าเวชภัณฑ์ ค่ายาที่ใช้ในการผ่าตัด ค่าทีมแพทย์ทำหัตถการ  เฉพาะที่เกี่ยวข้องกับการผ่าตัดตามที่ระบุเท่านั้น

6.ราคาดังกล่าวรวมอุปกรณ์พิเศษเช่น เลนส์ ที่ใส่ในร่างกายของผู้ป่วย (เฉพาะหัตถการที่กำหนด)

7.ราคาดังกล่าวรวมค่ายากลับบ้าน (Home Medication) แต่ไม่รวมค่ายาโรคประจำตัวของผู้ป่วย

8.ราคาดังกล่าวรวมการตรวจติดตามอาการหลังผ่าตัด 6 เดือน (จำนวน 5 ครั้ง)

วันหมดอายุ E-Coupon 

1. E-Coupon จะมีอายุการใช้ตั้งแต่วันที่ 28 ตุลาคม พ.ศ. 2566 - 31 มกราคม พ.ศ. 2567

2. หาก E-Coupon หมดอายุ รพ.ขอสงวนสิทธิ์การเข้าใช้บริการ และ E-Coupon ไม่สามารถขยายระยะเวลาในการใช้บริการหรือคืนเป็นเงินได้

3. ชุดตรวจสุขภาพราคานี้ สามารถสั่งซื้อได้ตั้งแต่ วันที่ 28 ตุลาคม พ.ศ. 2566 - 19 พ.ศ. 2566

หมายเหตุ

1. กรณีลูกค้ามีการขอยกเลิกคำสั่งซื้อหลังสิ้นสุด Shopee Guarantee โดยไม่ได้มีสาเหตุมาจากทางโรงพยาบาล โรงพยาบาลขอสงวนสิทธิ์ในการหักค่าดำเนินการ 10% จากราคาสุทธิในใบเสร็จ

2. ยอดเงินที่แสดงใบเสร็จเป็นยอดเงินที่ได้รับจากการชำระเงินจริงไม่รวมโปรโมชั่นส่วนลด

1.Service is reserved exclusively for customers who present the E-Coupon.

2.This E-Coupon cannot be exchanged for cash.

3.E-Coupon is only applied to specific items as mentioned and changes cannot be made.

4.These packages are for outpatients who are physically dependent. 

5.This price includes the cost of the operating room, recovery room, medical supplies, surgical medications, and surgical team fees specifically related to the specified surgery.

6.This price includes special equipment such as lenses that are implanted in the patients body (for specified procedures).

7.This price includes the cost of home medication but does not include the cost of regular medications for the patients existing conditions.

8.This price includes 6-month post-operative follow-up visits (a total of 5 visits).

10.This price is for Thai citizens and expatriates only.

Expiration Date of E-Coupon:

1. The E-Coupon is valid from October 28, 2023, to January 31, 2024.

2. If the E-Coupon expires, the hospital reserves the right to deny service access, and the E-Coupon cannot be extended or refunded.

3.This package price can be ordered from October 28, 2023, to November 19, 2023.

Note:

1. In case customers request cancellation of the order after the Shopee Guarantee period without valid reasons from the hospital, the hospital reserves the right to deduct a 10% processing fee from the net amount on the receipt.

2. The receipt amount displayed is the actual payment received, excluding any promotional discounts.
